| HFBWA Education Grant Form and Evaluation

Money is available to support events and activities which contain relevant goals and support the BWA’s educational purpose. It is made possible through funding from the BWA Federation.
The PDF form provides an outline for planning and reviewing activities. Submit it to FBWA Educational committee or unit president to apply for funding. |
| Requests for Payment | The first form is for printing out and submitting to the Treasurer in hardcopy.
The second version is a PDF form you can fill out electronically and send as an email attachment along with an image of your receipt(s). |
| HFBWA Proxy Form | A form by which a BWA officer, auditor, United BWA president, director, and/or minister advisor may designate another individual to act on their behalf at BWA meeting. |

| United BWA Request for Payment | This reimbursement request form (half-page size; two per page) can be used by any United BWA member. Fill in your own United’s name, attach your receipts and submit to your Treasurer. |
| United BWA Proxy Form | If applicable, any United BWA member may use this form to designate another individual to act on their behalf at a BWA meeting. Fill in your own United’s name. See example from Honolulu United’s policy. |
